Yes we are.
A home health care team comprises of physicians, registered nurses, licensed vocational nurses, certified home health aides, medical social workers and therapists who will collaborate and coordinate with you, your family and your other health providers in order to create a unique and personalized care plan just for you.
We will first interview you in order to find out what is the level of care that you need. We will then perform an assessment and evaluation to both parties in order to match an amiable patient – caregiver relationship.
This is a rare case for us because we often make sure that both parties are comfortable with each other before we have our caregiver start to work with our client. But in instances like this, we will reassess and reevaluate your needs and make proper arrangements to replace your caregiver as soon as possible.
As always, you can call us at our landline number, (714) 650-8519 where a representative will address your concerns.
